Intake, Waivers & Medical Release Forms

 You’re welcome to download and fill out the forms you’ll need for your first visit with us. If it’s easier, you can also complete everything when you arrive. Just plan to come about 15 minutes early so you have plenty of time to get settled and finish the paperwork. 

* If you’re new to Beach Life Wellness, please download, complete, and bring the "Intake & General Liability Waiver" to your appointment.

* Acupuncture (first visit): Bring the "Acupuncture Arbitration Agreement" along with the "Intake & General Liability Waiver".

* Advanced Esthetics: Bring the "Advanced Facials Pre/Post Care & Liability Waiver" plus the "Intake & General Liability Waiver".

* Exercise or Rehabilitation Programs: Bring the "Intake & General Liability Waiver" to your fitness assessment or first personal training session. If you’ve had any major heart conditions in the past or present, be sure to bring the "Physician Release Form" as well.
