Intake, Waivers & Medical Release Forms

If you are new to Beach Life Wellness you are welcome to download, complete and bring our 

"GENERAL LIABILITY WAIVER" form on the day of your treatment. 

If this is your first visit for an Acupuncture treatment, you are welcome to download and complete the 

"ACUPUNCTURE ARBITRATION AGREEMENT" in addition to the "GENERAL LIABILITY WIVER"

If you are trying to engage in an exercise/ rehabilitation program for the first time with us and have current, or past major heart conditions, 

you are welcome to download and complete the "PHYSICIAN RELEASE FORM", and bring it with you to your fitness assessment day, 

or first training/ rehabilitation session. 

You can also complete any of these forms upon arrival at our facility.
